At the  Center For Oral Health, we appreciate that you have selected our office to fulfill your oral health needs. We will try our utmost to provide you with the best possible care in a safe and gentle manner.

On your first visit, you will be requested to complete a written medical history. Please bring the names and dosages of all medications currently being used, the names and addresses of physicians and dentists from whom you are presently receiving care, and any pertinent insurance information.

During this visit, a thorough clinical evaluation will be conducted and any areas of special concern will be examined. Please bring any available dental x-rays. If none are available, we will take x-rays necessary to make an accurate assessment.

All findings and recommendations are fully discussed. Please allow approximately one hour for this initial visit.
